Here is the link to see the application form you need to request an exception from the smoking ban in Pennsylvania. Please remember several things: 1) Do not send the form regular mail. Send it registered so you can prove it was sent by the September 10 deadline. 2) If you are not sure if you qualify for the 20% exception, then check the appropriate box on Section V and use the Section IX revenue sheet and base your request on the upcoming six month sales. 3) If you do not have your original approval letter from the LCB, call 717-783-8250 and request one asap.
